Healthy Opportunities Pilot (HOP) Program - with Client Choice
The Healthy Opportunities Pilot program (HOP) was established and funded by the federal government and is administered at the state level, to test and evaluate the impact of providing food services and other non-medical interventions to high-needs Medicaid members who meet specific qualifying social risk factors, including food insecurity. Under a Medicaid 1115 Waiver Demonstration, state government agencies, such as the North Carolina Department of Health and Human Services, a key innovator in the HOP program, have appointed Network Leads who contract with local food pantries and other Human Service Organizations (HSO) to provide services to eligible Healthy Opportunity Pilot Medicaid enrollees.
SmartChoice’s Food is Medicine software plays a pivotal role in assisting HOP Network Leads, food pantries, food banks, and other Human Service Organizations in the provision of these critical HOP services to enrollees experiencing the qualifying social risk factor of food insecurity. In addition to the CRM functionality, Medicaid 1115 Waiver HOP enrollees can utilize SmartChoice’s online ordering platform to place online orders for the food itself (Produce Prescriptions, Healthy Food Boxes, Healthy Meals, and Medically Tailored Meals) with participating food pantries and other HSO’s. With SmartChoice, patients are provided with the empowerment and dignity of full client choice - where they can choose the specific foods they want, in accordance with the guidelines provided to them by their nutritionists or health care providers, rather than being given pre-selected boxes of food, much of which may not be to their tastes or liking. HOP enrollees can use personal mobile devices, computers, and even onsite kiosks to place orders. Food order kiosks can be conveniently positioned at the Network Lead’s location, participating healthcare provider offices, or the food pantry itself – making the entire process from HOP enrollment, to prescription, to food ordering completely seamless for all involved.
