Food & Nutritional Services Software Platform for Social Care Networks (SCN), Lead Entities, and Networked Community-Based Organizations (CBO) & other Health-Related Social Needs Services (HRSN) Providers
In January 2024 the Centers for Medicare and Medicaid Services (CMS) approved the New York Health Equity Reform (NYHER) Medicaid 1115 Waiver Demonstration, allowing for the establishment and funding of Social Care Networks (SCN) in New York State to deliver HRSN (Health-Related Social Needs) services to residents in need. As a result, New York state has established 9 regionalized Social Care Network Lead Entities which are each tasked with building a network of CBOs (Community-Based Organizations) and other organizations providing HRSN services withing their communities.
Healthy Food and Nutrition has been identified as one of the key HRSN and Health Equity Programs to be provided to Social Care Networks Medicaid 1115 Waiver eligible residents who are facing food insecurity or inequity in food availability or provision.
SmartChoice’s online food ordering and management software platform plays a pivotal role in assisting SCN Leads Entities, CBOs, and other organizations providing HRSN food services, such as local food pantries and food banks, in the provision of these critical Social Care Network services to program participants experiencing food insecurity or food inequity. All HRSN Food and Nutrition services are easily administered, tracked, reported, and re-imbursed via the SmartChoice Client Relationship Management (CRM) functionality
In addition to SmartChoice's CRM functionality, eligible residents can utilize SmartChoice’s online ordering platform to place online orders for the food itself with participating local food pantries and other CBOs and HRSN organizations. With SmartChoice, program participants and their households are provided with the empowerment and dignity of full client choice - where they can choose the specific foods they want, and where appropriate, in accordance with the guidelines provided to them by their nutritionists or health care providers, rather than being given pre-selected boxes of food, much of which may not be to their tastes or liking. SCN program enrollees can use personal mobile devices, computers, and even onsite kiosks to place orders. Food order kiosks can be conveniently positioned at the Social Care Network Lead Entities' location, participating healthcare provider offices, CBO, or the HRSN food pantry itself – making the entire process from SCN enrollment, to food prescription (where appropriate), to food ordering completely seamless for all involved.
Program participants can even choose from within the SmartChoice software interface, how they wish to receive their food orders: whether picked up in-person at the HRSN organization or food pantry; picked up at a more localized 24/7 accessible refrigerated locker; or delivered to their home.
